Exploration on the "Doctoring" training for eight-year medical education / 中华医学教育探索杂志

Zhejiang University began to recruit eight-year clinical students since 2005. Since then, Zhejiang University learned from the experience of foreign medical education, reformed and innovated and formed a distinctive curriculum system and training pattern for eight-year medical training, that named as "Doctoring". "Doctoring" is a course that focuses on the education of students' comprehensive quality and ability. Its main contents are the basic knowledge about human health, disease and medical development. With the use of advanced and scientific teaching concept and teaching mode, students were able to study the doctor-patient communication and interpersonal skills, and use medical knowledge and practice skills required for clinical work. And the students also had the opportunity to demonstrate the ability to solve problems in the foundation of life-long learning. "Doctoring" is more than a simple course for teaching medical professional knowledge.

The methodology validation and content assay of total polysaccharides in Pudi Enema / 药学实践杂志

Objective To establish the assay method for the total polysaccharide in Pudi Enema.Methods Phenol-sulfuric acid method was used for chromogenic reaction.The content of total polysaccharide was measured by UV spectrophotometry at 488.8 nm.Results The total polysaccharides calibration curve was at the range of 0~22.635 mg/L, with regression function being Y=0.062 06 X-0.003 34(r=0.999 8).The recovery of calycosin was 98.36%(RSD=2.34%).Conclusion This method is sensitive,rapid,accurate and reliable.It can be used to assay the content of total polysaccharide in Pudi Enema.

Extraction Optimization of Total Flavonoids from Oxytropis Falcata Bunge by Central Composite Design-response Surface Methodology / 中国药师

Objective: To optimize the extraction process of total flavonoids from Oxytropis falcata bunge.Methods: With the content of total flavonoids as the index,and based on the results of single factor tests, central composite design-response surface methodology was adopted to optimize the extraction technology with the solvent amount, ethanol concentration and extraction time as the factors.Results: The optimal extraction conditions were as follows: extracted twice with 20-fold amount of 66% ethanol, and 84 minute per time.Under the above conditions, the extraction rate of total flavonoids was up to 23.21 mg·g-1.Conclusion: The central composite design-response surface methodology is convenient and highly predictive for the extraction optimization of total flavonoids from oxytropis falcata bunge.

Correlation between optical density and colony forming units of Mycobac-terium tuberculosis suspension / 中国感染控制杂志

Objective To establish a reliable approach for quantification of colony forming unit(CFU)of Mycobac-terium tuberculosis (M.tb)by measuring optical density(OD).Methods M.tb suspension H37Ra was prepared using low-power ultrasonic or glass bead beating methods,and was two-fold serially diluted,OD at 600nm (OD600)of each dilution ratio was measured respectively,OD600 and dilution curve were analyzed to determine the optimum approach for preparing bacterial suspension,linear range of OD600,as well as linear relationship between OD600 and CFU.Results OD600 was 0.1 -0.6,linear regression analysis of OD600 and dilution ratio within linear range revealed that correlation coefficient (R2 )of glass bead beating and low-power ultrasonic methods were 0.98 and 1 .00 respectively,both presented a good correlation,low-power ultrasonic method was better than glass bead beat-ing method,bacterial suspension dispersed more evenly.Linear regression analysis results of OD600 and CFU val-ues showed that the regression equation of glass bead beating method and low-power ultrasonic method were CFU=2.35×107 ×OD600+4.42×105 and CFU=3.26×107 ×OD600+6.89×105 respectively.Conclusion Low-power ultrasonic method is a good method for preparation of M.tb suspension,combined the measurement of OD600 value, it can be a reliable and rapid method for quantitative analysis of M.tb.

Differences in clinical features between cholesteatoma in external auditory meatus and middle ear / 临床耳鼻咽喉头颈外科杂志

OBJECTIVE@#Differences in clinical features, especially facial nerve canal leision between cholesteatoma in external auditory meatus and middle ear were compaired.@*METHOD@#A retrospective clinical analysis was made. Clinical data included 125 cases of middle ear cholesteatoma with facial nerve canal leision and 28 cases of cholesteatoma occurred in external auditory canal from 2003-01-2014-08 in our hospital.@*RESULT@#Clinical course of cholesteatoma in external auditory canal was 4.97 ± 7.51 years, course of middle ear cholesteatoma was 16.60 ± 14.42 years (P < 0.01). 21 cases (75%) of external auditory canal cholesteatoma were manifested as pneumatic mastoid and 110 cases (88%) of middle ear cholesteatoma were manifested as diploic mastoid respectively. 22 cases (78.6%) of facial nerve canal damage-in mastoid segment in cholesteatoma of external auditory meatus and 76 cases (60.8%) of facial nerve canal damage in tympanic segment in cholesteatoma of middle ear were observed (P < 0.01). The incidence rate of ossicular errosion in middle ear chol-esteatoma was significantly higher than that in external auditory meatus (P < 0.01). The incidence of semicircular canal defects in middle ear cholesteatoma (30.4%), was significantly higher when comparing to the incidence (10.7%) in cholesteatoma of external auditory meatus (P < 0.05).@*CONCLUSION@#The site of facial nerve canal lesion in middle ear cholesteatoma and cholesteatoma of external auditory meatus were different. More attention should be paid before and during operation to avoid facial nerve injury, including physical examinations, especial otologic exams, radiological reading and careful operation.

Synergistic tumor suppression by Ad. RGD-iNG4 in human nasopharyngeal carcinoma cell CNE and its mechanism / 临床耳鼻咽喉头颈外科杂志

OBJECTIVE@#To investigate the effect of adenovirus-mediated ING4 with RGD on proliferation, apoptosis and cell cycle of human nasopharyngeal carcinoma cell CNE and explore its probable mechanism.@*METHOD@#CNE cells were infected with Ad. RGD-ING4 and adenovirus vector, ING4 gene expression level was detected by RT-PCR and the target protein expression was tested by Western blot. MTT assay was adopted to evaluate the efect of ING4 on cell growth of CNE, Annexin -V-PE/7-AAD Double staining was used to measure the efect of ING4 on apoptosis, and PI staining was used to measure the efect of ING4 on the cell cycle. Differential expression of P21, Bcl-2 and Bax gene was detected by RT-PCR,and Differential expression of Survivin and Caspase 3 protein was detected by Western blot.@*RESULT@#CNE cells were cultured with Ad. RGD-ING4 for 72 h ,the results showed that ING4 was overexpressed in CNE cells ,the growth of CNE cells was obviously inhibited , apoptosis rate was significantly increased and G2/M phase was arrested apparently. The results of RT-PCR showed that Ad. RGD-ING4 significantly down-regulated the Bcl-2 and up-regulates the Bax and P21 expression in CNE cells, and the difference was statistically significant(P < 0.01). Western blot showed that the expression of Survivin was decreased and Cleaved-Caspase 3 was increased.@*CONCLUSION@#Ad. RGD-ING4 can play the role of tumor suppressor synergies on nasopharyngeal carcinoma cell CNE by down-regulating Bcl-2, Survivin expression and up-regulating P21, Bax and Cleaved-Caspase 3 expression.

Patient safety monitoring indicators based on medical complaints / 中华医院管理杂志

Objective To build the indicators system to collect patient safety monitoring information, focusing on medical complaints. Methods With such methods as literature review and expert advice, building the system for medical complaints collection and monitoring. Such indicators are modified and improved in pilot operations. Results The framework of the medical complaint monitoring indicators system is built in five dimensions, comprising 8 grade-1 indicators including patient complaint causes and hospital cause analysis, and 20 grade-2 indicators. Conclusion These indicators are scientific and operable to detect adverse patient safety events.

Acusector Infrared and Pressed Needle Treat Spondylopathy / 浙江中医药大学学报

[Objective] To evaluate the clinical effect of acusector infrared and pressed needle on spondylopathy.[Method] Randomly divide the patients into treatment group 1 with acusector infrared and pressed needle,and control group 2 with acusector and infrared.[Result] In treatment group,8 cases were cured,32 better,5 not cured,and total effective rate 88.89%; for group 2,they were 2,28,15 and 66.67% respectively.There’s marked difference between them.[Conclusion] Acusector infrared and pressed needle are better in treating spondylopathy.

Diagnosis and treatment of the external auditory canal cholesteatoma invading the tympanic cavity and mastoid / 中国耳鼻咽喉头颈外科

OBJECTIVE To discuss the clinic presentation,imaging characteristics and surgical management of the external auditory canal cholesteatoma (EACC) with invasion of the tympaniccavity and the mastoid. METHODS This was a retrospective study including the clinical data of 14 cases of EACC invading tympanic cavity and mastoid from 1998 to 2003 . RESULTS All 14 cases showed different bone erosion in the four walls of external auditory canal with extention to the mastoid . Pars flaccida perforation were found in 4 cases with invasion of the cholesteatoma into the tympanic cavity, the ossicular chains were compressed or destroyed. The bony mastoid segment of facial canal was destroyed in 3 cases . Cerebral plate destruction and congenital stenosis of external auditory canal were found in one case respectively. According to the extent of disease, modified radical mastoidectomy was performed in 8 cases and radical mastoidectomy in 5 cases , meatoplasty in one case. The post-operative hearing of 10 cases with intact ossicula chains were normal.One case with compression,shift of ossicules also got normal audition after the operation. The hearing threshold of the air condition in 3 cases with ossicular disruptions got 15 to 20dB of improvement among the language frequency after reconstruction of ossicular chain. The 14 cases were followed-up from 18 months to 5 years and no recurrences were found. CONCLUSION The exact etiology of EACC still remains unclear. There is some difficulty in diagnosing external auditory canal cholesteatoma (EACC) invading the tympanic cavity and the mastoid because of its untypical clinic presentation . High-resolution temporal bone CT scan could help to find out the primary lesion and determine the range of the pathological changes, and choose the proper surgical approach.

MORPHOGENESIS OF THE HEART AND THE FORMATION OF THE EPICARDIUM OF CHICKEN EMBRYO / 解剖学报

The morphogenesis of the heart and the formation of the epicardium were studied with SEM, TEM and light microscopy. The results are as follows:1. There are two stages in the development of chicken heart: First is the formation of cardiac loop. In certain places, the sulcus appears outside, and the cresta appears inside during this stage. The formation of the cardiac loop indicates the establishment of the anlagen of the heart; Second is the differentiation of the anlagen, including the septation of the heart and the shift of each part of the heart. The shift of the atrium and the conus results from the shift of the right and left ventriculum. 2. The epicardium is not differcntiatcd from the myocardium, but formed from the mesenchymal cells which proliferate in front of the sinus venosus. These cells migrate and cover the entire surface of the myocardium.

THE MORPHOLOGICAL STUDY OF THE DEVELOPMENT OF ENDOCARDIAL CUSHION AND VENTRICULAR SEPTUM OF CHICKEN EMBRYO / 解剖学报

The development of the endocardial cushion and the ventricular septum of chicken heart was studied with SEM, TEM, light microscopy and histochemical methods. The results are as follows:1. The development of endocardial cushion begins from the proliferation of the endothelial cells which form cushion cells when these cells enter the cardiac jelly. Cushion cells also proceed mitosis and migrate toward the myocardium, and the endothelial cells cease proliferation when the reserve layers of cushion cells were formed. The fused endocardial cushion participates in the formation of atrioventricular orifice, the right tubercle prolongs toward the ventricular septum and eventually fuses with it.2. The cardiac jelly of endocardial cushion contains a great deal of hyaluronic acid which arranges in "trails", pioneering cushion cells migrate along the "trails" and change the component and arrangement of the matrix.3. Muscular ventricular septum is formed by aggregation of trabeculae inside the interventricular sulcus. The dorsal part of the ventricular septum fuses with the lower part of right tubercle of endocardial cushion and the ventral part fuses with the conus septum.
